The ratchet design is the straightforward one. You crank the handle, the mechanism steps along a notched shaft, and it holds where you leave it. There is no cam to set and nothing subtle happening: what you pull is what you get, and it stays got until you release it. For a lot of operations that directness is exactly what they want out of a calf jack at two in the morning. Against a cam-type puller, a ratchet advances in defined increments rather than continuously, which some people prefer because they can feel and count what they are taking up. It also means the mechanism runs on a toothed shaft, so keeping that shaft clean and free of grit is part of owning one. The tool is not the limiting factor in a hard pull; the operator's judgement is. A jack applies far more force than a person can, and it has no sense of when to quit. Decide in advance what your stopping point is and phone your veterinarian before you reach it, not after. Wash the shaft and mechanism after every use, dry it properly, and check the chains and straps before calving season rather than during it.
